Over the year advances have been made in the field of Chronic Pain Syndromes which include, Fibromyalgia, Restless Leg Syndrome, and many autoimmune conditions. While medicine has advanced in the field of technology, imaging, and laboratory testing, for many sufferers, the treatment has still been centered around a pharmaceutical “medication for everything “approach.
Instead of finding the root causes of our illnesses, medications simply treat the symptoms similar to what a bandaid does for a cut. A recent study shows that Americans consume over 70% of the world’s medications, yet we only make up approximately 3% of the world’s population. I don’t think that this comes as a surprise that Americans are getting sicker and sicker despite taking more and more medications. This is especially true as it relates to Chronic Pain and autoimmune conditions. In todays article, lets review the relationship between Gluten and chronic pain as well as the link between gluten and various autoimmune diseases. Lastly, I will explain functional medicine and its approach to treatment of both Chronic pain and autoimmune disease.

Chronic Pain and Gluten
Chronic pain lasts for weeks, months, or even years, and goes beyond the usual recovery period following an injury or illness. Common types include lower back pain, migraines, and arthritis. Importantly, it often resists conventional treatment, prompting individuals to seek innovative and personalized solutions. Chronic pain and autoimmune illnesses rank among the top health concerns that we often treat with drugs rather than investigating and searching for the underlying cause. These medications are able to, in some instances make life bearable but leave many patients wondering if there is a better way. Merely masking the symptoms can have serious side effects down the road. For many sufferers, the cycle goes on and on from one doctor to the next.
Research suggests that gluten could exacerbate chronic pain through inflammatory pathways. Gluten-related inflammation can potentially trigger or worsen conditions known for causing pain, such as fibromyalgia, rheumatoid arthritis, and migraines. For some, removing gluten from their diet has led to significant pain relief, thus highlighting the link between what we eat and how we feel.
Symptoms of Gluten Sensitivity and Chronic Pain
Alongside pain, symptoms of gluten sensitivity include digestive discomfort, brain fog, joint pain, and fatigue. Recognizing these symptoms early and understanding their connections might be crucial in adopting a diet that alleviates pain.
Gluten and Autoimmune Diseases
Gluten has been associated with several autoimmune diseases beyond celiac disease (*). While celiac disease is the most well-known autoimmune disorder directly linked to gluten, other conditions have also shown varying degrees of association with gluten sensitivity or intolerance. Here are some autoimmune diseases that have been linked to gluten:
1. Dermatitis Herpetiformis and Gluten :
This is a skin condition characterized by itchy, blistering skin, and it is directly associated with celiac disease. Patients who have dermatitis herpetiformis almost always experience an improved condition with a gluten-free diet. In patients who haveDermatitis Herpetiformis, the ingestion of gluten leads to the deposition of Immunoglobulin A (IgA) antibodies in the skin. This deposition is what causes the characteristic rash and blistering in people with DH.
For those diagnosed with Dermatitis Herpetiformis, adherence to a strict gluten-free diet is essential—and highly effective (*) Eliminating gluten from the diet can lead to significant improvements in skin lesions and alleviate itchiness, sometimes providing visible results in a matter of weeks. However, complete healing of the skin and intestines can take several months to years, depending on the individual and the strictness of the dietary adherence.(*)
2. Hashimoto’s Thyroiditis and Gluten:
Hashimoto’s Thyroiditis, or Hashimoto’s disease, is characterized by the immune system attacking the thyroid gland. As a result, the thyroid’s ability to produce hormones diminishes, which can cause symptoms like fatigue, weight gain, cold intolerance, and depression. It is one of the most prevalent autoimmune diseases worldwide and the leading cause of hypothyroidism.
While not all individuals with Hashimoto’s have celiac disease, research has suggested that there may be increased gluten sensitivity among people with Hashimoto’s. Gluten sensitivity is a less understood and more variable condition than celiac disease, as it involves adverse reactions to gluten consumption but without the intestinal damage typical of celiac disease.
Molecular mimicry occurs when the immune system mistakes gluten proteins for thyroid tissue, potentially increasing the attack on the thyroid gland. This exacerbate inflammation and may worsen thyroid function.
Moreover, people with Hashimoto’s often report digestive issues, joint pain, and fatigue, symptoms that overlap with those of gluten sensitivity. Consequently, addressing gluten in the diet becomes a consideration in managing these overlapping issues.
Many individuals with Hashimoto’s Thyroiditis find symptom relief through a gluten-free diet(*). Adopting a gluten-free diet has been shown to reduce inflammation, improve gut health, and, in turn, influence thyroid autoimmunity (*)
3. Type 1 Diabetes and Gluten:
The connection between Type 1 diabetes and non-celiac gluten sensitivity speaks to the complexities of autoimmune disorders and immune responses to dietary components.
There appears to be a higher prevalence of celiac disease among individuals with type 1 diabetes than in the general population, suggesting that a subset of patients may benefit from a gluten-free diet (*). People with both Type 1 diabetes and gluten sensitivity often report overlapping symptoms, such as gastrointestinal discomfort or systemic inflammation, which can complicate the management of their diabetes. This makes it crucial for those with T1D to pay close attention to dietary trigg
ers that could exacerbate symptoms. Many patients with T1D and NCGS who adopt a gluten-free diet report improvements in symptoms like digestive discomfort and increased energy levels. Going on a gluten free diet requires more than jus swapping out your regluar cookies for gluten free cookies. Many gluten free foods have a very high glycemic index which could cause your blood sugar levels to spike. It’s essential for individuals with T1D to consult a Certifed Functional Medicine Practitioner before major dietary changes to ensure nutritional adequacy and proper diabetes management.
4. Rheumatoid Arthritis and Gluten:
Rheumatoid arthritis (RA) is a chronic autoimmune condition characterized by inflammation and pain in the joints. For those managing RA, understanding dietary factors like gluten and its role in chronic inflammation can be crucial. Recent research has increasingly shown that depending on the foods we eat, gut flora may be affected by an inflammatory or anti-inflammatory response, thus playing an important role in inflammatory autoimmune diseases, such as rheumatoid arthritis or gastroenterological disorders.
Autoimmune diseases often have overlapping genetic and inflammatory pathways. Although RA and celiac disease manifest differently, they may share common autoimmune triggers, including gluten.
Researchers speculate that one way Gluten sensitivity contributes to RA is partly due to an increase in gut permeability caused by gluten. When the gut lining becomes more permeable (leaky), it allows larger molecules to enter the bloodstream, triggering systemic inflammation and exacerbating RA symptoms. Individuals with rheumatoid arthritis report reduced inflammation and symptoms when minimizing or eliminating gluten from their diets (*). Patients implementing a gluten-free diet sometimes report improvements in fatigue levels and quality of sleep, which can result in a better quality of life overall.
While a gluten-free diet may not be a universal solution for everyone with RA, it holds potential benefits for a subset of patients who are gluten-sensitive. Personalized diet plans developed in consultation with healthcare providers can lead to better management of RA symptoms and an improved quality of life.(*)
5. Multiple Sclerosis (MS)and Gluten :
Multiple sclerosis (MS) is a chronic disease of the central nervous system that can lead to a range of symptoms, such as muscle weakness, coordination issues, and fatigue. With autoimmune dynamics playing a critical role in the progression of MS, there is growing interest in understanding the impact of dietary choices on managing this condition.
Avoiding wheat could lessen inflammation and alleviate MS symptoms. However, as I have mentioned in many other articles, it is not only the gluten in wheat that may cause issues. In some people, other proteins in wheat can cause inflammatory or allergic reactions, which may be just as serious.
Before going gluten-free, testing for celiac disease and for non celiac gluten sensitivity can help identify if this strict dietary change is medically necessary. An accurate diagnosis can guide dietary recommendations and management strategies.
6. Systemic Lupus Erythematosus (SLE) and Gluten :
Lupus, particularly systemic lupus erythematosus (SLE), is a chronic autoimmune disease characterized by widespread inflammation and tissue damage involving the skin, joints, kidneys, and other organs. Identifying factors that can modulate disease activity or symptom burden is crucial for individuals managing lupus.
Both lupus and NCGS share symptoms such as digestive distress, fatigue, and joint pain. In patients with lupus, sensitivity to gluten might exacerbate these symptoms, leading to increased severity or frequency of lupus flares.
Gluten sensitivity, even when not rising to the level of celiac disease, might contribute to immune activation that can influence lupus disease activity. As with all autoimmune disorders, responses to gluten may vary widely among individuals with lupus. What triggers a response in one person might not affect another. This is why I cant stress enough the importance of testing.
Patients report reduced symptoms such as fatigue and joint pain when avoiding gluten, potentially improving overall quality of life. For those who are sensitive, a gluten-free diet may lead to reductions in systemic inflammation, potentially dampening some autoimmune responses.
7. Psoriasis and eczema:
Psoriasis and eczema (also known as atopic dermatitis) are chronic inflammatory skin conditions that affect millions of people worldwide. Both conditions are marked by episodes of itching, redness, and discomfort. Many individuals with gluten sensitivity report exacerbation of skin conditions, including psoriasis and eczema, upon gluten consumption.
Some studies indicate a link between gluten sensitivity and psoriasis and eczema.
A Better Way to Treat Chronic Pain and Autoimmunity
Thankfully, there is a better way to treat chronic pain and autoimmunity rather than relying on medication. One of the approaches that is making headway has been in the field of neuroimmunology. Simply put, this is how the immune system and nerve system are interconnected. An area of special interest to many researchers and clinicians including myself is the effect certain foods have on the immune system. Gluten, in particular, the protein found in wheat, barley, malt, and rye, oats is one of the culprits behind chronic pain and autoimmune illnesses (*),(*).
I can state from my personal and clinical experience that, for patients who suffer from chronic conditions, If gluten is eliminated for 90 days there is often a drastic improvement. By identifying Gluten sensitivity as well as other food sensitivities you can easily treat, manage and improve Chronic Pain Syndromes, fibromyalgia, Restless Leg Syndrome, and many autoimmune conditions through diet.
Functional Medicines Approach to Chronic Pain: Strategies for Relief
