Patient Responsibility to Disclose Health Information

Working With Other Healthcare Providers

This includes any supplements, medications, protocols, testing, diets, detox programs, antimicrobials, hormones, thyroid medication, antibiotics, probiotics, binders, peptides, injections, or other treatments you are using.

Not knowing what other treatments you are receiving can result in duplicated recommendations, conflicting protocols, adverse reactions, or worsening symptoms.

Dr. Hagmeyer cannot be responsible for outcomes related to treatments, protocols, supplements, medications, or recommendations provided by other practitioners.

Supplements, Herbs, and Product Use

Supplements, herbs, vitamins, minerals, probiotics, enzymes, binders, antimicrobials, detoxification support, and other products may affect individuals differently.

Some individuals may experience reactions including, but not limited to:

Specialty functional medicine laboratories may use different testing methods, reference ranges, reporting formats, and interpretation models than conventional medical laboratories.

Lab interpretation provided by Dr. Hagmeyer is not a diagnosis and does not replace evaluation by your primary care physician, specialist, or prescribing healthcare provider.

Testing Refund Policy

Once a lab test has been ordered, processed, submitted to the lab, shipped, electronically generated, or placed with the laboratory, it is non-refundable.

If testing is part of your case review recommendations and the test has been ordered or placed with the lab, the testing fee is non-refundable.

You are responsible for completing lab testing within the required time frame and following all instructions provided by the laboratory.

DrHagmeyer.com is not responsible for delays caused by the laboratory, shipping carriers, specimen collection errors, incomplete samples, rejected samples, expired kits, patient delays, or failure to follow test instructions.

Lab Results and Medical Follow-Up

If your lab results contain abnormal, critical, or concerning findings, you are responsible for following up with your primary care physician, specialist, urgent care, or emergency care provider when appropriate.

Dr. Hagmeyer’s review of functional or wellness-related lab markers does not replace medical evaluation or treatment.

No Guarantee of Results

Every individual is different.

Orders

Supplement orders are generally fulfilled Monday through Thursday.

Orders placed after 4:00 p.m. Eastern Time on Thursday may not be processed or shipped until the following Monday. Our office does not typically fulfill orders on Friday, Saturday, Sunday, or holidays.

Shipping times may vary based on product availability, carrier delays, weather, holidays, supply issues, manufacturer backorders, or other circumstances outside our control.

Payment Methods and Billing

If you purchase products, services, testing, consultations, programs, or other items through DrHagmeyer.com, payment may be made by credit card, debit card, PayPal, or any other payment method we may accept at our discretion.

Product Returns

Unopened merchandise purchased from DrHagmeyer.com may be returned within 7 days of purchase, subject to approval.

All approved returns are subject to a 15% restocking fee, merchant fees from Stripe of 3.5%.

Opened products are not refundable.

DrHagmeyer.com reserves the right to reject any return that does not comply with these requirements or in cases where abuse, repeated returns, improper storage, tampering, or misuse is suspected.

Damaged, Lost, or Delayed Shipments

If your order is damaged during shipping, you must notify our office within 24 hours of delivery.

You will be required to provide:

• Lot Number on the bottle
• Delivery confirmation
• Carrier information
• Written description of the damage

You may also be required to file a claim with the shipping carrier, including USPS, UPS, FedEx, or another carrier.

DrHagmeyer.com is not responsible for carrier delays, lost packages, stolen packages, incorrect shipping addresses, weather delays, or delivery issues once the package has been transferred to the carrier.

Please make sure your shipping address is accurate before placing your order.

Services and Refunds on Services

Services purchased through DrHagmeyer.com must be used within 90 days unless otherwise stated in writing.

Unused services, including case reviews, consultations, programs, or service credits, are non-refundable after 90 days.

If a service expires, DrHagmeyer.com may, at its discretion, offer a merchandise credit or service credit toward another product or service. Such credit is not guaranteed and may be subject to restrictions.

Case Reviews and Missed Appointments

If you fail to attend your scheduled Low, Moderate, or High Complexity Case Review, phone consultation, video consultation, or appointment, a rescheduling fee may be required to reserve a new appointment time.

Currently, the rescheduling fee for a missed case review is $395 unless otherwise stated.

Appointments are reserved specifically for you. Missed appointments prevent other patients from using that time.

Cancellation Policy

I understand that appointments are reserved specifically for me and that missed appointments prevent other patients from using that time.

I understand that all canceled appointments require at least 72 hours’ notice, and appointments canceled with less than 72 hours’ notice may be charged the full fee for the time reserved.

I understand that not receiving an appointment reminder does not waive cancellation, no-show, missed appointment, or rescheduling fees.

I understand that voicemail, email, text message, or portal message does not automatically cancel or reschedule an appointment unless cancellation or rescheduling has been confirmed by the office or completed through an approved scheduling system.

I understand that email, text, voicemail, and portal messages are intended for brief administrative questions, scheduling, billing, order updates, and simple clarification only.

I understand that detailed health questions, symptom changes, lab interpretation questions, supplement reactions, treatment-plan questions, or new concerns may require a scheduled consultation.

I understand that No-shows may be charged the full appointment fee and/or a rescheduling fee.

I understand that DrHagmeyer.com reserves the right to refuse future scheduling for repeated cancellations, no-shows, unpaid balances, or failure to comply with office policies.

Treatment Plans and Professional Time

Once Dr. Hagmeyer has begun reviewing your case, developing a treatment plan, reviewing records, interpreting testing, communicating with you, coordinating with staff, or preparing recommendations, professional time has been reserved, and work has begun.

If you terminate or cancel a treatment plan after work has begun, any refund or remaining credit may be calculated after deducting professional time, administrative time, testing review, email correspondence, chart review, consultation time, and communication with other healthcare professionals.

Dr. Hagmeyer’s professional hourly rate is currently $600 per hour unless otherwise stated. This rate is subject to change without notice.

If you have questions about fees, hourly rates, cancellation terms, or refund calculations, please contact our office before purchasing or beginning a treatment plan.

Chargebacks and Payment Disputes

By purchasing products, testing, services, consultations, programs, or treatment plans from DrHagmeyer.com, you agree to contact our office first to resolve billing, refund, shipping, or service concerns before initiating a chargeback or payment dispute.

Initiating a chargeback, payment dispute, or payment reversal does not cancel, waive, or eliminate responsibility for services, appointments, lab testing, supplement orders, treatment plans, administrative work, professional time, or fees that were authorized, provided, ordered, shipped, reserved, or incurred.

If a chargeback or dispute is submitted before allowing a reasonable time for a return, refund, exchange, or billing review to be processed, a chargeback fee of $100 may be deducted from any amount owed or credited.

DrHagmeyer.com reserves the right to pursue unpaid balances, collection costs, administrative fees, legal fees, and other remedies permitted by law.

I agree to contact DrHagmeyer.com first to resolve any billing, refund, shipping, service, testing, or order concern before initiating a chargeback, payment dispute, or payment reversal.

I understand that initiating a chargeback or payment dispute does not cancel, waive, or eliminate responsibility for services, appointments, lab testing, supplement orders, treatment plans, administrative work, professional time, or fees that were authorized, provided, ordered, shipped, reserved, or incurred.
