Alternative: Try the green part of a leek. Using the green part of leeks provide a similiar taste wtihout the high levels of fructans you would get in onions.

8. Beer and Alcohol

Do you need a reason? Beer and alcohol can be problematic for individuals with methane SIBO for several reasons:

  1. Carbohydrate Content: Beer contains fermentable carbohydrates that can serve as a substrate for bacteria and archaea in the small intestine. This fermentation can lead to increased production of methane gas and exacerbate symptoms like bloating and constipation.

9. Cheese (Soft Cheeses)

Reason: Soft cheeses like ricotta and cottage cheese tend to have higher lactose content and are often restricted on a low FODMAP diet. Cheese can be problematic on a low FODMAP diet for several reasons. In the context of SIBO, the main problem is its lactose content. Lactose is a disaccharide, and it is the “D” in FODMAP component  that can trigger symptoms in people who are lactose intolerant or lactose sensitive. But it’s also crucial to recognize that cheese can impact gut health beyond its lactose content.

The presence of casein, a protein found in cheese, can also affect individuals with certain sensitivities, potentially leading to digestive discomfort. Additionally, some cheeses may contain additives or preservatives that can irritate the gut lining or disrupt the balance of beneficial bacteria. What about mold you say? While mold is a natural part of certain cheeses, individuals with mold allergies may experience reactions when consuming these cheeses. The primary concern is the ingestion of mold spores, which can trigger an allergic reaction in susceptible individuals. 

It’s important to note that individual tolerance varies, so some people may be able to consume larger quantities of cheese without symptoms, while others might need to avoid it completely. Artificial sweetners and Methane SIBO

10. Low Sugar/Zero Calorie Sweeteners-“Polyols” and Artificial Sweeteners

Polyols, also known as sugar alcohols, can exacerbate symptoms of small intestinal bacterial overgrowth (SIBO) and irritable bowel syndrome (IBS) because they are poorly absorbed in the small intestine and ferment in the large intestine, leading to gas and bloating. A low FODMAP diet, often recommended for managing SIBO and IBS, restricts polyols due to their potential to worsen symptoms.

When it comes to polyols you will need to start reading labels and ingredients in the foods you eat. Overall, while polyols offer a lower-calorie alternative to sugar, they can pose significant challenges for those with SIBO or IBS.  Studies have also shown that Polyols as well as artificial sweeteners like Sweet & Low, Aspartame, Sucralose, saccharin, AceK cause intestinal dysbiosis(*),(*),(*). Here are some common types of zero or low-calorie sweeteners that contain polyols:

  1. Sorbitol: This is often used in sugar-free candies, chewing gum, and baked goods. Sorbitol is about 60% as sweet as sucrose (table sugar) and contains approximately 2.6 calories per gram. It can cause digestive issues like bloating and diarrhea when consumed in large amounts.
  2. Mannitol: Similar to sorbitol, mannitol is used in sugar-free products, including mints and some medications. It has about 50-70% of the sweetness of sugar and contains about 1.6 calories per gram. Excessive consumption can lead to gastrointestinal discomfort.
  3. Xylitol: Xylitol is a popular sugar substitute found in sugar-free gum, mints, and oral care products. It is about as sweet as sugar but has 40% fewer calories, providing around 2.4 calories per gram. It is also known for its dental health benefits but can cause digestive upset if consumed in large quantities.
  4. Erythritol: This is a unique sugar alcohol that is almost zero-calorie (about 0.2 calories per gram) and is about 70% as sweet as sugar. Erythritol is often used in low-calorie and sugar-free products, and it is well-tolerated by most people, as it does not usually cause digestive issues at typical consumption levels. However, some individuals may still experience mild digestive discomfort.
  5. Isomalt: Often used in sugar-free candies and chocolates, isomalt is about 45-65% as sweet as sugar and contains about 2 calories per gram. It is known for its ability to withstand high temperatures, making it suitable for baking. As with other polyols, excessive consumption can lead to digestive issues.
  6. Maltitol: This polyol is commonly used in sugar-free products, including chocolates and baked goods. Maltitol is about 75-90% as sweet as sugar and contains approximately 2.1 calories per gram. It can cause digestive discomfort, such as gas and bloating, when consumed in large amounts.
  7. Lactitol: Used in certain sugar-free foods, lactitol is about 30-40% as sweet as sugar and contains approximately 2 calories per gram. It is often found in ice creams and baked goods. Like other polyols, it can lead to digestive discomfort if consumed excessively.

Alternative: Maple syrup, coconut sugar, Stevia? its a bit qustionable in my opinion. “Several studies, mainly in laboratory animals, have identified potential side effects of stevia over the last few decades. Stevia metabolism is dependent upon gut microbiota and microbial enzymes can break down these glycosides into steviol that can be absorbed by the host. However, the effects of stevia on the gut microbiota need to be further studied(*).

This is not an exhaustive list of every food that may need to be avoided for Methane SIBO management, but it’s a great starting point. Each person’s sensitivity to certain foods may vary, so it’s important to work with a healthcare provider to customize your diet for optimal results.

Your diet can make or break your SIBO situation, but you also need to keep in mind that getting rid of methane SIBO requires more than just avoiding foods. The large intestine houses a significant portion of the gut microbiome, and imbalances here can impact methane gas production and gut health.

It’s about strategically starving those methane-producing troublemakers as well as healing a leaky gut, improving your secretory IgA levels, improving stomach acid and digestion, increasing the levels of your short-chain fatty acids, and more.

Can a Low FODMAP diet, SCD diet, or Elemental diet alone treat or cure SIBO? NO! But for some, it can provide great relief while the root cause of the overgrowth is addressed. For some, it’s like hitting the reset button on your gut.

1. SCD Diet For SIBO

The Specific Carbohydrate Diet (SCD) is another dietary approach used during treatment for SIBO. The Specific Carbohydrate Diet (SCD) was originally developed to help manage conditions like celiac disease, Crohn’s disease, and ulcerative colitis. The diet is based on the idea that certain carbohydrates can be poorly absorbed by the intestines, leading to various digestive issues. By focusing on easily digestible foods, the SCD aims to reduce intestinal inflammation and promote a healthy gut environment.

The SCD may be beneficial for some individuals with SIBO because it focuses on eliminating foods that can ferment in the gut and contribute to bacterial overgrowth. By reducing the intake of certain carbohydrates, the diet may help reduce symptoms like bloating, gas, and diarrhea, which are common in SIBO.

2. Elemental Diet For SIBO

The Elemental Diet is a specialized dietary regimen that consists of easily digestible, specific nutrients in their simplest forms. It is often used in the treatment of various gastrointestinal conditions, including Small Intestinal Bacterial Overgrowth (SIBO), and comes in the form of liquid shakes or powders that can be mixed with water. Think of it as predigested food for your gut. The Elemental Diet typically contains pre-digested proteins that are easily absorbed, simple carbohydrates such as glucose or maltodextrin, which require minimal digestion, and Fats in the form of medium-chain triglycerides (MCTs).

FODMAP stands for fermentable oligosaccharides, disaccharides, monosaccharides, and polyols — which are types of carbohydrates that can be poorly absorbed in the small intestine and fermented by gut bacteria.

SIBO Treatment Protocol: Beyond Diet

When it comes to IBS or SIBO treatment, you will need to make modifications to your diet. A low FODMAP diet is crucial, but it’s not enough to treat SIBO once you have it. It’s only one piece of a much bigger puzzle. Also keep in mind that Restricting FODMAPs for too long a period of time, has the potential to cause nutritional deficiencies as well as harmful changes in the types of bacteria growing in the gut (*). Antimicrobial therapy is often used to address the overgrowth of archaea in the small intestine, with typical treatment durations varying depending on the severity of the overgrowth.

Probiotics for SIBO? Yes, but it’s a bit complicated. Some help, some can increase hydrogen, methane, or Hydrogen sulfide gas.

Antibiotic treatments like Rifaxamin, Erythromycin, and Zyfaxan can have side effects and impact our good bacteria, and for many people, SIBO relapse rates are very high. This is partly due to the fact that treating SIBO is more than just killing bacteria. I explain this and much more in this article. 

Antimicrobial treatments? Yes, but some are more powerful than others. If you have low levels of Firmicutes or low levels of Bacteroidetes, you and your doctor may not want to rush in with antimicrobials or antifungals right away. These bacteria account for 95% of the bacteria that make up the gut microbiome. While Antimicrobials and antifungals are important, they too should be used judiciously on intestinal methanogens (IMO)- Methane-producing gut microorganisms.

But here’s the kicker: you’ve got to address the root cause(s) of SIBO.

I’m going to assume that you already had a SIBO breath test, but for those who stumbled on today’s article, A SIBO breath test is like a breathalizer for your gut(*).

A breath test is a diagnostic procedure used to determine whether there is an overgrowth of bacteria in the small intestine. The test typically involves measuring the levels of hydrogen and/or methane in the breath after ingestion of a specific substrate, often lactulose or glucose. If you have High methane levels on your SIBO breath test, that’s an indicator of methane SIBO (*). Some patients can actually have more than one type of SIBO at the same time. Its not uncommon for a patient to have a combination of methane SIBO, Hydrogen SIBO, and Hydrogen Sulfide SIBO.

When to get tested? If you’re bloated, constipated, and feeling full after meals, it’s time to get a SIBO breath test.
