Low Testosterone and SIBO
Small Intestinal Bacterial Overgrowth, or SIBO, is a condition characterized by an abnormal increase in the population of bacteria in the small intestine. Unlike the large intestine, where bacteria are beneficial and plentiful, the small intestine typically has relatively low bacterial counts. When these bacteria proliferate excessively in the small intestine, they can interfere with the digestion and absorption of nutrients, leading to various gastrointestinal symptoms.
Common symptoms of SIBO include bloating, gas, abdominal pain, diarrhea, and malnutrition, but this is just the start! There are many other Symptoms of SIBO that are not as obvious.
When you have an excess of these bacteria they can ferment carbohydrates before they are digested, leading to the production of gas and discomfort. SIBO is diagnosed through either a lactulose breath test or a Glucose Breath test.

Low Testosterone Linked To Severity and Onset of IBS and SIBO symptoms
Men with low testosterone are at increased risk of chronic inflammation and its related disorders, such as Irritable Bowel Syndrome (IBS).(*)
Recent studies have found a compelling link between low levels of testosterone and an increased risk of developing IBS.(*) More notably, these studies also demonstrate that men with reduced testosterone levels may experience more severe symptoms associated with the condition. This new research reveals that men suffering from IBS have decreased levels of free testosterone, which may contribute to the severity of their symptoms.
Why Testosterone Is Important For Gut Health.
Testosterone levels are important for both men and woman and the interplay between testosterone and gut health is a fascinating area of research that has garnered increasing attention in recent years. Testosterone, a vital hormone primarily associated with male health but also present in females, plays a crucial role beyond just reproductive functions—it impacts various physiological systems, including the gastrointestinal (GI) tract. Low testosterone levels have been linked to several gut-related issues, such as Small Intestinal Bacterial Overgrowth (SIBO), impaired GI motility, and increased abdominal pain.
In my practice, I often talk about the BIG picture when it comes to helping people who struggle with chronic health issues. Very often in Functional Medicine, we need to take a step back, and really start to look at the connections one part of the body has on another. Understanding the relationship between testosterone and gut health, might not seem important for doctors who practice conventional medicine, but for me and many other Functional Medicine Practitioners around the world, understanding what affects motility, helps us have a more comprehensive understanding of the problem but also how to tackle it.

Testosterone and the Gut Microbiome
The gut microbiome is a complex ecosystem of trillions of microbes that play a vital role in digestion, immune function, and overall health. Abnormal testosterone levels can drastically alter the composition of bacteria living in your gut. A balanced testosterone level fosters a diverse and stable gut microbiome, while imbalances are a contributing factor that lead to intestinal dysbiosis or SIBO.
Studies have shown that low testosterone levels can decrease microbial diversity, promoting the growth of harmful bacteria at the expense of beneficial ones. This imbalance can contribute to conditions like SIBO, Leaky gut, Irritable Bowel Syndrome (IBS), and even metabolic disorders like diabetes and thyroid disorders. On the other hand, excessively high testosterone levels can also disrupt microbial equilibrium, causing similar health issues. Remeber its always about balance.

Testosterone and Gut Barrier- Leaky Gut
Testosterone helps maintain the integrity of the mucosal barrier in the gut, which is crucial for keeping harmful bacteria at bay. When testosterone levels are low, this barrier can become compromised, making the gut more susceptible to bacterial overgrowth.
Research indicates that both chronic and acute inflammation, triggered by endotoxin exposure, are linked to testosterone deficiency. Endotoxins are among the particles that can pass through the gut lining in cases of leaky gut syndrome. In particular, inflammation in men can interfere with the function of Leydig cells in the testes, which are essential for testosterone production.
An imbalance in gut bacteria can lead not only to Small Intestinal Bacterial Overgrowth (SIBO) but also to a leaky gut.
Testosterone influences the smooth muscle function in the gut.(*) Proper motility, or the movement of food and waste through the GI tract, depends on the coordinated contractions of these muscles. Low testosterone can impair these contractions, leading to slowed GI transit times—a condition known as gastroparesis. This can result in symptoms such as constipation, bloating, and discomfort, creating a cycle of digestive inefficiency.
Testosterone has anti-inflammatory properties that help reduce inflammation in the gut. Lower levels of testosterone can elevate the body’s inflammatory response, exacerbating conditions like Irritable Bowel Syndrome (IBS) and other inflammatory GI diseases. Chronic inflammation can cause abdominal pain, and cramps, and even contribute to more severe conditions such as inflammatory bowel disease (IBD).
Understanding the connections between low testosterone and gut health is essential for developing more effective treatment plans. Addressing hormonal imbalances could be a crucial component in managing GI disorders that stem from or are exacerbated by low testosterone levels. Low Testosterone Abdominal Pain and Bloating

Low testosterone levels often correlate with heightened gut sensitivity, exacerbating perceptions of pain and discomfort. This is why some people may experience more intense symptoms of IBS or other gastrointestinal disorders when their testosterone levels are low.(*)
Testosterone and Bile Acid Production
Bile is a digestive fluid that aids in the breakdown and absorption of fats. It is produced in the liver and stored in the gallbladder. Testosterone plays a significant role in regulating bile production. (*) Optimally balanced testosterone levels ensure adequate bile flow, promoting efficient digestion and nutrient absorption.
Low testosterone can hinder bile production, leading to fat malabsorption, which in turn can cause steatorrhea (fatty stools) and deficiencies in fat-soluble vitamins like A, D, E, and K. High testosterone levels, however, may result in overproduction of bile, increasing the risk of conditions such as gallstones and bile acid diarrhea. (*)
The Importance of Bile When You Have SIBO
Small Intestinal Bacterial Overgrowth (SIBO) is a condition where an excessive number of bacteria accumulate in the small intestine.(*) While the gut microbiome should be densely populated, this is not true for the small intestine, which should have relatively fewer bacteria. Bile plays a vital role in managing SIBO due to its antimicrobial properties.
Adequate bile production helps maintain a balanced microbial population by neutralizing excess bacteria in the small intestine. When testosterone levels are imbalanced, as discussed earlier, bile production might be affected, exacerbating SIBO symptoms. Ensuring proper bile flow is crucial for those dealing with SIBO, and maintaining balanced testosterone levels becomes integral to this process. (*),(*)
