You can have thyroid symptoms, a “normal” TSH, and still feel like your body is running in low power mode. Fatigue, brain fog, cold hands and feet, stubborn weight changes, hair shedding, and feeling wired but exhausted often push people to ask how to support reverse T3. That question matters, but the real answer is rarely about forcing one lab marker down. It is about understanding why your body is shunting thyroid hormone in that direction in the first place.

What reverse T3 Actually Means
Reverse T3, often written as rT3, is an inactive form of thyroid hormone. Your body makes it from T4, the storage form of thyroid hormone. Under the right conditions, T4 gets converted into active T3, which helps regulate metabolism, energy, temperature, digestion, and brain function. But under other conditions, more T4 can get converted into reverse T3 instead.
This is not automatically a bad design flaw. It is part of the body’s survival response. When your system senses stress, inflammation, under-fueling, illness, or physiologic strain, it may slow down energy output. Reverse T3 can rise as part of that adaptation.
That is why treating reverse T3 like the problem itself often misses the bigger picture. In functional medicine, we ask why the body is making that shift. The goal is not just to chase a number. The goal is to restore the environment that allows healthier, optimal thyroid conversion.

If you want to know how to support reverse T3, start with the factors that influence conversion from T4 to active T3. In many cases, the body is prioritizing survival over performance. It depends on the person, but a few common patterns show up again and again.
Stress Physiology Matters More Than Most People Realize
Chronic stress is one of the biggest drivers behind poor thyroid conversion. This includes emotional stress, overtraining, blood sugar swings, sleep deprivation, chronic pain, infections, and hidden inflammation. Your body does not separate these stressors neatly. It responds to the total load.
When stress hormones stay elevated or become dysregulated over time, your body may reduce active T3 production and make more reverse T3. This can be one reason people feel depleted even when standard thyroid labs look acceptable.
Supporting stress physiology does not mean telling someone to simply relax. It means improving sleep quality, stabilizing blood sugar, reducing excessive exercise when needed, supporting recovery, and identifying chronic triggers that keep the nervous system stuck in a defensive state.
Inflammation Can Shift Thyroid Hormone Conversion
Inflammation is another major reason reverse T3 may rise. This can come from gut dysfunction, autoimmune activity, chronic infections, poor blood sugar control, food reactions, toxin exposure, or unresolved metabolic issues. If inflammation is ongoing, the body often adapts by dialing down thyroid activity at the tissue level.
This is where context matters. A person with Hashimoto’s, chronic digestive symptoms, and fatigue may need a very different plan from someone whose reverse T3 shifted after extreme dieting or a recent illness. The lab pattern may look similar, but the root causes are not.

Under-eating and restrictive dieting can backfire
One of the most overlooked causes of high reverse T3 is not eating enough. When the body senses that food is scarce, it becomes very smart about conserving energy. Calorie restriction, very low-carbohydrate diets, prolonged fasting, skipping meals, and inconsistent eating patterns can all send the same message: resources are limited.
When that happens, the body may downshift metabolism as a protective response. Instead of converting thyroid hormone efficiently into active T3, the body may shift more toward reverse T3, an inactive form of thyroid hormone that essentially slows things down. This is not the body “failing.” It is the body adapting to perceived stress, scarcity, or survival demand.
This is why some patients feel worse the harder they diet. They may be eating clean, cutting carbs, fasting longer, and exercising more, yet their fatigue, cold hands and feet, hair shedding, constipation, poor recovery, and stubborn weight gain continue. In many cases, the issue is not a lack of discipline. The issue is that the body no longer feels safe enough to run a strong metabolism.
Many patients restrict food with good intentions. They are trying to lose weight, reduce inflammation, improve blood sugar, calm gut symptoms, or follow a plan that worked well for someone else. But what works for one person can backfire in another, especially when there is already thyroid dysfunction, adrenal stress, gut inflammation, nutrient deficiency, chronic infection, mold exposure, or a long history of dieting.
Very low-carb diets can be especially problematic for some people with thyroid conversion problems. Carbohydrates are not just “sugar.” They influence insulin, leptin, cortisol, liver glycogen, and thyroid hormone conversion. Research has shown that fasting and calorie restriction can reduce active T3 and increase reverse T3, while carbohydrate availability can influence T3 production.
This does not mean everyone needs a high-carbohydrate diet. It also does not mean fasting, keto, or low-carb diets are always bad. The point is that these strategies are tools, not universal solutions. If your reverse T3 is elevated and your symptoms suggest a slowed metabolism, more restriction may not be the right next step.
For some people, improving reverse T3 starts with eating enough food consistently. That may mean getting adequate protein at each meal, bringing in enough carbohydrates to support thyroid conversion and adrenal function, avoiding long gaps without food, and making sure total calories are not chronically too low.
It also means looking at the bigger picture. If the body is underfed, inflamed, overtrained, underslept, or constantly stressed, thyroid conversion will often reflect that. You cannot force the thyroid to work optimally in a body that feels like it is in survival mode.
In root cause medicine, the goal is not to chase reverse T3 with a medication or supplement. The goal is to ask why the body is making more reverse T3 in the first place. Sometimes the answer is inflammation. Sometimes it is poor gut health, blood sugar instability, low iron, chronic stress, or infection. And sometimes, it is simply that the person has been under eating for too long.
Supporting thyroid conversion requires creating the conditions for the body to feel safe again. That includes enough nourishment, enough rest, stable blood sugar, adequate protein, appropriate carbohydrates, and a plan that matches the patient’s physiology rather than forcing the body into deeper restriction.
There is no perfect macro split for everyone. But if your body is conserving energy, the answer is rarely to keep eating less and pushing harder. Sometimes the first step toward better thyroid conversion is giving the body the message it has been waiting for: food is available, energy is coming in, and it is safe to stop slowing everything down.

Key Areas to Investigate
A reverse T3 issue is rarely isolated. It usually shows up as part of a broader pattern. That is why a root-cause evaluation should look beyond one marker.
Blood sugar regulation
Blood sugar instability is a hidden stressor. If you are skipping meals, crashing in the afternoon, waking at 3 a.m., or depending on caffeine to function, your body may be compensating more than you realize. Repeated highs and lows can contribute to cortisol disruption, inflammation, and thyroid conversion issues.
Balanced meals with protein, fiber, and healthy fats often help. Some people also need to reduce refined carbs, improve insulin sensitivity, or address more advanced metabolic dysfunction.
Gut Health and Absorption
The gut plays a central role in thyroid health. Poor digestion, microbiome imbalances, constipation, intestinal inflammation, and nutrient malabsorption can all interfere with the body’s ability to make, convert, and respond to thyroid hormone.
If you have bloating, reflux, food sensitivities, irregular bowel movements, or a history of antibiotics, there may be more going on than a thyroid-only problem. In many cases, improving gut function changes the entire picture.

Nutrient status
Thyroid conversion depends on adequate nutrients. Selenium, zinc, iron, magnesium, iodine, vitamin A, and B vitamins all matter. So does overall protein intake. If you are deficient, your body may struggle to convert T4 to active T3 efficiently.
Iron deserves special attention, especially in women with heavy periods, low ferritin, fatigue, and hair loss. You can have a thyroid pattern that looks sluggish when the deeper issue is poor oxygen delivery and low iron stores. This is why guessing with supplements is not ideal. Testing helps you target the right deficiency instead of adding random products.
Liver health and detox burden
A meaningful amount of thyroid conversion happens in the liver. If liver function is impaired by fatty liver, toxin load, alcohol, medications, poor diet, or chronic inflammation, thyroid metabolism can suffer. This does not mean everyone with reverse T3 has a liver disease issue. It does mean your liver deserves attention as part of the conversation.
Illness, infection, and immune activation
Acute illness and chronic infections can raise reverse T3. So can autoimmune activity. If the immune system is constantly activated, the body often shifts resources away from energy production and repair. This is one reason some people do not improve until hidden infections, mold exposure, or autoimmune drivers are addressed.
Labs that Give Better Context
If you are trying to understand how to support reverse T3, testing should not stop with TSH alone. A complete thyroid workup may include free T4, Total T4, total T3, free T3, reverse T3, both thyroid antibodies, and markers related to inflammation, iron status, blood sugar, nutrient levels, and cortisol patterns.
No single lab tells the whole story. Reverse T3 can be helpful in context, but it should not be interpreted in isolation. A number on paper means very little if no one is asking why your body created that pattern.

What Support Can Look Like in Practice?
For many people, improving reverse T3 is less about a quick fix and more about removing physiologic roadblocks. That may mean eating enough, sleeping more consistently, supporting blood sugar, calming inflammation, restoring nutrient status, improving gut function, or changing an exercise routine that has become too stressful for the body.
Sometimes thyroid medication needs to be reevaluated. Sometimes it does not. That depends on your symptoms, full lab picture, history, and how well your body is converting hormones. This is where individualized care matters. Two people can have similar symptoms and need completely different strategies.

The Biggest Mistake People Make
The biggest mistake is trying to micromanage reverse T3 without addressing the stressors that caused it. People often search for one supplement, one thyroid medication adjustment, or one dietary trick to bring the number down. But if your body is still under-fueled, inflamed, sleep-deprived, or dealing with a hidden burden, the pattern usually persists.
Your body is not broken. It is adapting. The real work is understanding what it is adapting to.
If you have been told your labs are normal but you know something is off, keep asking better questions. Reverse T3 is often a clue that your system needs deeper support, not a signal to give up on finding answers.
