New Patient Application and Getting Started
Congratulations on taking the first step toward better understanding your health.
The way Dr. Hagmeyer works with patients is different from the conventional medical model many people are accustomed to. Our office focuses on a holistic, Functional Medicine approach that looks beyond symptoms alone and considers how multiple systems in the body may be contributing to your health concerns.
Dr. Hagmeyer does not prescribe medications, and he does not replace your Primary Care Physician or other medical specialists.
Instead, his role is to take a closer look at the bigger picture by reviewing your health history, symptoms, previous testing, medications, supplements, nutrition, lifestyle, and appropriate laboratory findings.
Dr. Hagmeyer believes in testing, not guessing.
The information you provide in your New Patient Application is an important part of this process. Please take your time and be as thorough as possible.

Why Your New Patient Paperwork Is So Important
Your symptoms are only one part of your health story.
The information you provide helps Dr. Hagmeyer understand:
- When your symptoms began
- How your health has changed over time
- Previous diagnoses
- Medications you currently take or have taken in the past
- Supplements you are using
- Previous treatments
- Dietary patterns
- Digestive symptoms
- Hormonal symptoms
- Thyroid symptoms
- Sleep and stress patterns
- Previous laboratory testing
- Lifestyle and environmental factors
The more complete the information you provide, the better Dr. Hagmeyer can understand the history and context of your case.
Please Complete Your 7-Day Food Journal
Your New Patient paperwork includes a 7-Day Food Journal.
Please begin this as soon as possible and record your food and beverage intake as accurately as you can.
Your food journal can provide valuable information about dietary patterns, meal timing, food choices, and other nutritional factors that may be relevant to your health.
Please include the completed journal with your New Patient paperwork.
What Happens After Your Application Is Submitted?
Once your New Patient Application has been received, your file will be reviewed to make sure the information needed to move forward has been provided.
The next steps generally include:
- Completion of your New Patient Application
- Completion of the testing recommended by Dr. Hagmeyer
- Case Review
- Development of your treatment plan
- Follow-up testing to assess progress and make appropriate adjustments
This structured process allows Dr. Hagmeyer to begin your evaluation with both a detailed health history and objective laboratory information.

Testing Comes Before Your Case Review
Dr. Hagmeyer uses laboratory testing to establish an objective baseline before completing your Case Review.
This is an important part of his approach.
Rather than making recommendations based on symptoms alone, testing provides additional information that can help identify patterns involving areas such as:
- Thyroid function
- Blood sugar and insulin regulation
- Inflammation
- Hormone balance
- Nutritional status
- Iron status
- Cardiovascular health
- Liver and kidney function
- Blood cell and immune patterns
- Digestive health when appropriate
The goal is to gather the information Dr. Hagmeyer needs before your Case Review so that your consultation can focus on understanding your results and discussing the next steps.
Do You Have Recent Testing From Another Provider?
If you have recent laboratory or functional medicine testing from another provider, please let us know.
Dr. Hagmeyer will review one previous test from another provider, provided the testing was completed within the last three months.
If your testing is current and includes information Dr. Hagmeyer needs for your evaluation, it may not be necessary to repeat that particular test.
Testing that is more than three months old may still provide useful historical information, but Dr. Hagmeyer does not use older results as the basis for current treatment recommendations, since laboratory values and health patterns can change over time.
Please submit any recent testing you would like considered when completing your New Patient Application.
What Your Case Review Includes
Before your Case Review, Dr. Hagmeyer reviews the information you have provided along with the testing required for your initial evaluation.
Your Case Review may include:
- Review of your New Patient Application and health history
- Review of your primary health concerns and typed health narrative
- Review of your 7-Day Food Journal
- Review of your medication and supplement history
- Review of your Metabolic Assessment
- Review of your Neurological Assessment, when applicable
- Review of your completed bloodwork and other testing
- Review of important patterns identified within your history and laboratory findings
- Consultation with Dr. Hagmeyer to discuss your results and recommended next steps
The goal of the Case Review is to bring the different pieces of your health history together.
Rather than looking at one symptom or one laboratory result in isolation, Dr. Hagmeyer looks for relationships and patterns across multiple systems.
Dr. Hagmeyer Reviews Your Case Before Your Appointment
Dr. Hagmeyer reviews your file before your scheduled Case Review appointment.
This allows your appointment time to be spent discussing the information that matters most rather than using the consultation simply to collect your history.
By the time you meet with Dr. Hagmeyer, he has already had an opportunity to review your health history, laboratory findings, previous testing, medications, supplements, dietary information, and other relevant details.
This preparation is an important part of Dr. Hagmeyer’s Big Picture approach.
Functional Medicine Looks at the Bigger Picture
Functional Medicine recognizes that the body’s systems do not operate independently.
Your:
- Thyroid
- Hormones
- Blood sugar
- Digestive system
- Immune system
- Liver
- Nutritional status
- Inflammatory pathways
- Nervous system
can all influence one another.
Dr. Hagmeyer’s approach is designed to consider these relationships and determine which patterns may be most important in your case.
Recommendations may include nutrition, nutritional supplementation, natural approaches, lifestyle changes, and patient education.
The goal is to help you better understand your health and give you a clearer path forward.
We Are Selective About the Cases We Accept
Dr. Hagmeyer accepts a limited number of new patients so that sufficient time can be dedicated to reviewing each case and providing personalized care.
New Patient Applications are reviewed in the order they are received.
Submitting complete paperwork and testing helps prevent unnecessary delays in the review process.
Please be as thorough as possible when completing your application.
Incomplete information may delay the next step in your evaluation.
