When this ecosystem is out of balance, symptoms can show up almost anywhere in the body—and routine testing often misses the real problem.
That’s where a functional stool test comes in.
Instead of simply looking for obvious infections or blood in the stool, functional stool testing uses advanced DNA technology to map your gut microbiome, screen for hidden pathogens, measure digestive enzymes, and evaluate markers of inflammation and leaky gut. In my practice, this type of advanced stool test is one of the most powerful tools I use to uncover the root causes behind chronic digestive issues and many seemingly “mysterious” health problems.

What a Functional Stool Test Can Reveal
Pathogens and Opportunistic Organisms
A functional stool test like a GIMAP stool test also screens for a wide range of gastrointestinal pathogens, including:

Bacterial pathogens such as Salmonella, Shigella, Campylobacter, and pathogenic E. coli

Helicobacter pylori, often with virulence factors (e.g., CagA, VacA) that help estimate risk for ulcers, gastritis, and gastric cancer

Steatocrit / fecal fat – evaluates fat digestion and absorption. Elevated values suggest fat malabsorption, which can cause loose stools, abdominal discomfort, and deficiencies in fat-soluble vitamins.
When I see low elastase or elevated fecal fat on a functional stool test, I may recommend targeted digestive enzyme support, bile support, and dietary changes to improve nutrient absorption rather than just treating symptoms.
Inflammation, Immunity, and Leaky Gut
Functional stool testing can also assess inflammation and mucosal immunity, often using markers such as:

Calprotectin – helps differentiate between functional issues like IBS and more serious inflammatory bowel disease (IBD). Elevated calprotectin suggests active inflammation that may require further workup.

For intestinal permeability (leaky gut), some tests offer zonulin as an add-on marker. Elevated zonulin levels suggest increased intestinal permeability, which may contribute to:

In many cases, no fasting or special preparation is required, unless your practitioner advises temporarily holding certain supplements or medications that can skew results.

Because the sample is preserved, it usually remains stable for several days at room temperature, allowing for flexible shipping.
Completed kits are shipped back to the laboratory (often via prepaid mailer or FedEx Clinical Pak). Lab processing for a functional stool test generally takes 7–14 business days, depending on the test and workload.
