If you have been told your labs are normal, but your body feels anything but normal, you are not imagining it. The best tests for chronic fatigue are rarely limited to a basic CBC and thyroid screen. Fatigue is a symptom with many possible drivers, and if you stop testing too early, you often stop answers before they start.
That is one of the biggest problems with the conventional workup for fatigue. It is often designed to rule out disease, not explain why you are dragging through the day, waking unrefreshed, crashing after meals, or feeling like your brain and body are moving through wet cement. A standard blood panel (CMP and CBC) that comes back “normal” does not mean nothing is wrong. One of the most overlooked areas in fatigue cases is incomplete thyroid testing. Many patients are told their thyroid is fine because their TSH falls within range. That is not always enough. A more meaningful thyroid evaluation often includes free T4, free T3, reverse T3, and thyroid antibodies such as TPO and thyroglobulin antibodies.
This matters because you can have thyroid-related fatigue even when a single marker appears acceptable. Some people are cant convert T4 thyroid hormone into T3. Others have autoimmune thyroid patterns that are affecting energy, metabolism, mood, and body temperature before obvious thyroid disease is diagnosed.
If you are exhausted, cold, gaining weight easily, losing hair, or dealing with constipation, broader thyroid testing deserves attention.

Cortisol and Stress Physiology Can Be Part Of The Picture
When stress has been chronic, sleep has been poor, and the nervous system has been stuck in survival mode, the body often pays a price. This does not mean fatigue is just stress. It means stress biology may be one of the mechanisms driving it.
A diurnal cortisol saliva test, often done with saliva or urine, can help show whether cortisol output is too high, too low, or poorly timed across the day.
